County HealthCare Center hopes to ease the choices that must be made.
Bent County HealthCare Center's programs restore hope to a wide range
of individuals - from those who need help as they recover from an
injury or illness to those with permanent disability or loss of
function who need long term care.
Bent County HealthCare Center is a non-profit, county-owned facility.
We have 60 beds and accept a variety of payer sources: Medicare,
Medicaid, VA certified, Long-term Care Insurance and Private Pay.
Our
strengths are the community participation in our programs, our
excellent intergenerational programs, and our commitment to improving
the quality of care each year. We provide you with excellent customer
service and can answer your questions in a confidential, respectful
manner.
Our elders and families participate in our internal
operations by providing valuable feedback to Administration.

Our Bent County
HealthCare Center Team believes in an interdisciplinary
approach that focuses on the whole person; physical,
emotional, spiritual and social. Our team of professionals,
staff, and consultants provide a comprehensive range of
services for elders in the skilled nursing facility or
assisted living.

MEDICARE CERTIFIED SKILLED SERVICES:
We provide inpatient specialized nursing and
rehabilitation services for patients who no longer
require hospitalization, but due to their medical or
physical condition, cannot return home. We provide
intensive physical, occupational and speech therapy;
24-hour nursing care and psychosocial programs
individualized in meeting the needs of each patient. The
goal is assisting patients in achieving their highest
level of functioning so they can return to their home. |

LONG-TERM NURSING HOME CARE: We provide
interdisciplinary care that focuses on meeting the
physical and psychosocial needs of each elder. Our
philosophy focuses on assisting each resident to
maintain an optimal level of self-care and independence.
Families of our residents play an important part of our
"team" and are encouraged to visit often and participate
in developing the individual plan of care. |

ALZHEIMER'S SPECIAL CARE UNIT: We provide
specialized care for elders with Alzheimer's and
other memory loss diseases. Our staff is specially
trained to understand the disease and to assess the
elder's needs so that we can provide nursing care,
therapeutic activities and interventions that are
designed to best meet the needs of each elder. |
